Download New Client Forms: Client Intake Form and Treatment Guidelines

New clients:  You can schedule your appointment online or by phone. You will be emailed Client Intake Forms, which are also available for download here. Please complete these forms and review the Treatment Guidelines (included with the forms). You may also fill out the forms at the spa at the time of your appointment.  Please come in 10-15 minutes before your scheduled appointment time, so that your therapist can review your paperwork and conduct an assessment before your massage.

Cancellation policy: Please allow at least 24 hours for changes and cancellations for services 75m and under and 48 hours for changes and cancellations for services 90 minutes and over.  If you have a late cancellation, you may be charged for a portion or full amount of the service. We understand that last minute emergencies and illness do occur and you will not be charged in these instances.

Lateness: All Appointments are allotted with a generous amount of time before and after the treatment, so that the client can ask questions and the therapist can conduct a thorough assessment.  We do ask that new clients arrive 10-15 minutes before their appointment and repeat clients arrive 5 minutes before.  We understand sometimes lateness does occur but if you continually late or you are over 5 minutes late, we may need to deduct the time from your session. Full price will be charged.
